Wrist and Forearm Pain in Musicians and Repetitive Tasks

I once worked with a Baroque recorder player who came to see me with thumb, wrist, and forearm pain.

Now, for those of you who do not know what a Baroque recorder is — neither did I initially. They are a considerable step up in sophistication from the plastic recorders many of us were handed in elementary school music class.

What struck me most was not only the technical complexity of the instrument, but also the physical demands required to play it at a high level:

  • prolonged thumb positioning

  • sustained fine motor control

  • repetitive finger movement

  • upper body endurance

  • breathing coordination

  • prolonged postural demands

Like many musicians, this individual had already tried resting and reducing playing time, yet the symptoms persisted.

This is a very common presentation.

Many people expect wrist and forearm pain to come from one isolated structure or one “overuse” injury. In reality, these symptoms are often influenced by a much larger system involving:

  • repetitive loading over time

  • endurance and recovery capacity

  • neck and shoulder positioning

  • scapular stability

  • trunk and core support

  • nerve sensitivity

  • cumulative stress and performance demands

Often, symptoms develop gradually long before pain becomes noticeable.

The Hand and Wrist Do Not Work Alone

Although symptoms may be felt most strongly in the thumb, wrist, or forearm, the upper extremity functions as part of a larger connected system.

The hand and wrist rely heavily on:

  • shoulder blade (scapular) stability

  • upper back positioning and endurance

  • trunk control

  • core stability

  • cervical and thoracic mobility

  • coordinated movement throughout the entire kinetic chain

When the larger supporting regions fatigue or lose efficiency over time, smaller distal structures such as the forearm, wrist, and hand may begin compensating.

For musicians and individuals performing repetitive fine motor activity, these compensations can gradually increase loading demands through already highly active tissues.

This is one reason why wrist and forearm pain is not always simply a “local wrist problem.”

Symptoms Often Build Gradually

Many individuals notice symptoms beginning as:

  • tension or tightness

  • forearm fatigue

  • heaviness in the hands or arms

  • reduced endurance

  • aching after activity

  • stiffness the following morning

  • intermittent tingling or sensitivity

Over time, symptoms may begin appearing earlier during activity or persist longer afterward.

Some people initially notice symptoms only during demanding tasks, while others eventually feel discomfort during:

  • typing

  • texting

  • driving

  • carrying objects

  • prolonged desk work

  • household activities

  • gripping or lifting

This gradual progression is common in repetitive strain presentations.

Understanding Nerve Irritation and Double Crush Syndrome

Some individuals also experience:

  • tingling

  • numbness

  • burning sensations

  • heaviness

  • altered sensation

  • symptoms extending into the hand or fingers

In certain cases, nerve sensitivity may be influenced by more than one area along the nerve pathway.

This concept is sometimes described as double crush syndrome.

For example, a nerve may already be experiencing increased sensitivity or mechanical stress around:

  • the neck

  • upper thoracic region

  • shoulder girdle

  • forearm or wrist

When multiple regions contribute to reduced nerve tolerance simultaneously, symptoms may become more persistent or easily aggravated.

This is one reason why assessment often needs to look beyond only the wrist or hand itself.

Why Rest Alone Does Not Always Solve The Problem

Many people become understandably cautious once symptoms begin.

They may stop:

  • practicing

  • exercising

  • lifting

  • sports participation

  • repetitive activities altogether

While temporary reduction of highly aggravating activities may sometimes help calm irritable symptoms, prolonged complete rest often does not fully restore the body’s ability to tolerate repetitive demands long term.

In some cases, too little loading may contribute to:

  • reduced endurance capacity

  • decreased strength tolerance

  • increased sensitivity once activity resumes

  • ongoing fear around movement or playing

For many repetitive strain conditions, rehabilitation focuses on gradually rebuilding tolerance and endurance rather than avoiding activity completely.

The Role of Endurance and Sustained Loading

Forearm and wrist symptoms are often less about maximal strength and more about sustained endurance capacity over time.

This is particularly relevant for:

  • musicians

  • students

  • desk workers

  • gamers

  • writers and artists

  • healthcare professionals

  • trades and repetitive occupations

Many repetitive tasks involve prolonged low-level muscular activation without enough variation or recovery.

Remaining in one position or repeating similar movements for extended periods — even with technically “good” posture — may still contribute to muscular fatigue and tissue sensitivity over time.

Musicians and Performance Demands

Musicians often face unique physical and emotional demands.

Periods of increased symptoms commonly occur during:

  • auditions

  • juries and exams

  • orchestra rehearsals

  • recording projects

  • performance preparation

  • learning technically demanding repertoire

I have worked with musicians from organizations including the Vancouver Symphony Orchestra, the Vancouver Academy of Music, university music programs, adult learners, and recreational performers balancing music alongside work and family responsibilities.

Many musicians are highly disciplined and accustomed to pushing through discomfort, which sometimes means symptoms are ignored until they become more persistent.

What Physiotherapy May Focus On

Assessment and rehabilitation may involve:

  • identifying contributing loading and movement factors

  • assessing endurance, mobility, and strength capacity

  • evaluating scapular, trunk, and postural control

  • assessing nerve mobility and sensitivity

  • improving pacing and recovery strategies

  • supporting gradual return to playing and activity tolerance

Treatment may also incorporate:

  • exercise-based rehabilitation

  • endurance and loading progression

  • movement retraining

  • education around load management and recovery

  • manual therapy

  • intramuscular stimulation (IMS) where clinically appropriate

These approaches are integrated when clinically appropriate as part of an individualized rehabilitation plan.

Recovery Is About More Than Reducing Pain

For many people, recovery involves:

  • restoring confidence with movement

  • improving endurance capacity

  • returning to meaningful activity

  • reducing fear around repetitive use

  • improving long-term tolerance to work, music, sport, and daily activity

Many individuals are relieved to learn that symptoms do not necessarily mean permanent damage or the need to stop meaningful activities altogether.

With appropriate rehabilitation and gradual progression, many people are able to return successfully to music, work, exercise, and everyday life.

Practical Strategies That May Help

For many individuals, symptoms improve with:

  • pacing repetitive activity more effectively

  • incorporating regular movement variability

  • improving endurance and strength gradually

  • improving scapular and trunk support capacity

  • balancing workload and recovery

  • reducing prolonged sustained gripping or positioning

  • improving overall physical conditioning and activity levels

Small sustainable adjustments are often more effective than dramatic changes or complete avoidance.
