Why Musicians Develop Repetitive Strain Injuries

Last week, a local pianist came to see me for bilateral forearm pain that had been ongoing for approximately three months.

He had already tried resting from both piano playing and his sport but had not noticed meaningful improvement.

What was particularly frustrating for him was that the symptoms did not seem to make sense.

At the time, he was actually away from school and no longer practicing the same intensive hours he had been several months earlier. He reported no major changes in technique, no new equipment, and no clear injury that started the pain.

However, he was becoming increasingly concerned because he anticipated a heavier workload approaching, with new material to learn for upcoming rehearsals and performances.

This type of presentation is very common among musicians.

Symptoms often appear confusing because repetitive strain injuries are rarely caused by one single event alone. Instead, they are often influenced by a combination of:

Professional and recreational musicians alike may spend hours practicing without realizing how much cumulative physical demand is occurring over time.

Symptoms Often Develop Gradually

Most repetitive strain injuries do not begin with one obvious injury.

The nervous system and musculoskeletal system often respond together during these periods of cumulative stress and loading.

Why Rest Alone Is Not Always the Solution

Many musicians become fearful that movement or playing itself is harmful.

Some individuals stop playing entirely for prolonged periods hoping symptoms will completely disappear.

While temporary reduction of aggravating activity may sometimes help calm irritable symptoms, complete avoidance often does not fully restore the body’s ability to tolerate playing demands long term.

For many musicians, rehabilitation focuses on:

  • gradually rebuilding playing tolerance

  • improving endurance and movement capacity

  • reducing excessive tension and guarding

  • improving pacing and recovery strategies

  • supporting confidence with performance and movement again

The goal is often not simply resting more, but improving the body’s ability to tolerate meaningful activity more consistently.

What Physiotherapy May Focus On

Assessment and rehabilitation may involve:

  • understanding instrument-specific demands

  • identifying contributing movement and loading factors

  • assessing endurance, mobility, and strength capacity

  • addressing prolonged positioning and repetitive strain patterns

  • improving pacing and recovery strategies

  • supporting graded return to practice and performance

Treatment may also incorporate:

  • exercise-based rehabilitation

  • movement retraining

  • manual therapy

  • education around load management and recovery

  • intramuscular stimulation (IMS) where clinically appropriate

These approaches are integrated when clinically appropriate as part of an individualized rehabilitation plan.

Recovery Is About More Than Reducing Pain

For many musicians, recovery is deeply connected to identity, creativity, performance, and emotional well-being.

Rehabilitation is often about:

  • restoring confidence with playing

  • improving endurance and tolerance

  • reducing fear around symptoms

  • supporting sustainable long-term performance

  • maintaining participation in meaningful artistic activity

Many musicians are relieved to learn that symptoms do not necessarily mean they must stop playing permanently.

With appropriate rehabilitation and gradual progression, many people are able to continue performing and participating meaningfully in music.
